United Spinal and the Permobil Foundation have partnered to offer United Spinal members the opportunity to apply for a Roll Forward grant of either a Smart Drive or a Active-Height seat elevator on a new Permobil wheelchair order. Awards are limited and will be given based on financial need.
Obviously, you must already have insurance or medicaid/Medicare willing to buy you a new wheelchair, but this can help if you have that and can demonstrate financial need. Joining United Spinal is quite easy, free, and can be done online.
Manual chair users who have medicare can usually get the Smart Drive anyway, so I suspect this is more geared to private insurance users. Anyway, the process for applying is simple and explained here:
This is so awesome and I wished it had been, or that I had been aware of this, before I got my chair. I needed to prove it was medically necessary to my insurance (same as medicare/medicaid rules) and though I had everyone possible provide documentation I lost my case in appeal. I had to concede that technically it was not medically necessary but it makes life better for me; access to the sink and cook stove, makes transfers possible and most important, makes it possible to steer while fishing. I will save this for next time . Thanks!
