After 40+ years of SCI I have first hand experience with SCI pain and also have many friends who have provided me with their second-hand experiences. This is a very tricky issue. I have great empathy for all of you who suffer, especially during the day. The pain interferes with work, play, and any other activity that allows us to function independently.
However, my pain has always been tolerable during the day, but starts at bedtime and interferes with sleep. Sleep deprivation is akin to torture over an extended period. Besides, if you can't sleep during the night, you end up sleeping during the day, and that also interferes with daily activities.
About 25 years ago my SCI doc put me on Darvocette. I would save all four tablets for the night. It was miraculous and I returned to normal sleep patterns. Unfortunately, about three years ago, all kinds of black box warnings were published regarding long term use of Darvocette. My doc than put me on Tramadol. Tramadol is non-narcotic and theoretically not physically addictive. Believe me, if something helps control spinal pain, it becomes addictive, whether it's physical or mental. Long story short I got the same result with Tramadol with lower dosage than the Darvocette. I had never heard of Tramadol before this. If you have uncontrolled spinal pain, it's certainly worth a trial if your doc recommends it. The narcotic stuff is dangerous in so many ways and many of us get in trouble with prolonged use of narcotic pain killers.
hobo, thanks for sharing this. I have used Tramadol during times of unusual pain, and also found it effective. Another benefit is that it doesn't turn BMs to stone, as opiates do (risk of impaction). Fortunately, do not need Tramadol on regular basis yet.
I agree that loss of sleep is very destructive. Because of chronic steroid use to control my disease, I am not able to sleep without meds. So far, this formula has worked for me for almost 6 years without increase in dosage: about an hour before bed, 1020mg valerian root (like camomile tea without having to urinate!), and at bedtime 10mg zolpidem tartrate (brand is Ambien, not CR version) and 10mg melatonin. In USA, in 2015 FDA will limit Rx of zolpidem tartrate to 90 doses a year; if my doctor cannot justify continuing the protocol that has worked so well for me, I will join your rank. Prediction: after an extended period without regular and sufficient sleep, I will become psychotic and the stress of this will exacerbate my disease and hasten my death.

Tramadol does work well for me in combination with Lyrica. If I stay at a low amount my head clears a bit as does my vision. I can function so-so on 150mg of both a day. I guess you have to realize you have to live with more discomfort, and Drive On.
Great to hear from so many of you. No matter what others may say, there can be safer solutions to SCI pain. However, it is definitely a matter of trial and error. Tramadol just happens to work for me. Don't give up.
