Once you become Spinal Cord Injured and admitted to your rehab center/hospital you will normally be scored under the ASIA grading.
This is a neurological assessment involving a pin prick test to determine sensory levels and a motor examination to determine muscle strength.
You will be graded as ASIA A, B, C, D or E
What does the grading mean?
A- Complete No motor or sensory function in the lowest sacral segment (S4-S5)
B- Incomplete Sensory function below neurologic level and in S4-S5, no motor function below neurologic level
C- Incomplete Motor function is preserved below neurologic level and more than half of the key muscle groups below neurologic level have a muscle grade less than 3.
D- Incomplete Motor function is preserved below neurologic level and at least half of the key muscle groups below neurologic level have a muscle grade >3
E- Normal Sensory and motor function is normal
Sensory levels are graded as: 0 – The sensation is absent 1 – The sensation is present but imapired 2 – The sensation is normal
Muscle strength is graded as 0 - Total paralysis 1 - Palpable or visible contraction 2 - Active movement, full range of motion, gravity eliminated 3 - Active movement, full range of motion, against gravity 4 - Active movement, full range of motion, against gravity and provides some resistance 5 - Active movement, full range of motion, against gravity and provides normal resistance [Muscle able to exert, in examiner’s judgement, sufficient resistance to be considered normal if identifiable inhibiting factors were not present] NT – not testable. Patient unable to reliably exert effort or muscle unavailable for test-ing due to factors such as immobilization, pain on effort or contracture.
i was under the impression that ASIA A is defined as having no Anal sensation, i discussed this with dj too. The score criteria might have changed. Again i took it as having no motor or sensation below injury level. I refer to myself as a T11 ASIA A above that everything is normal and even more.

delboy..the grading has not changed..the Sacral segment (S4-S5) does in fact refer to rectal sensation....S5 represents the skin immediately at and next to the anus.
I cannot understand this ASIA grading thing at all. I asked my consultant in October when I saw her if I was still an ASIA A SCI, and she said yes I was. Where I get confused about the grading is I can move my legs, walk all below the level of my injury. Yes I have no bowel or bladder control and my bum is totally numb, but I can walk, with the aid of a walking frame, so thats why I am so confused on this matter.

Butiki, its no wonder you are confused....no you cannot be ASIA A.....because, as you say, you can move your legs and to the degree of walking with aids.
Therefor, you are either ASIA C or ASIA D....i would guess you are C at the moment but could potentially be D if strength improves to your muscles below injury.
I am ASIA D and i suspect that i am stronger than you at this moment.
From the Cauda Equina Syndrome perspective you sound Complete which because i know you retain (urine retention) so you are probably CESR which means you have a complete syndrome in respect of CES.

I am in a similar boat to you and having checked with several spinal doctors they have all told me that the absense of sacral sparing (ie sensation or motor function around the butt) comes ahead of everything else so we are indeed ASIA A.
However we clearly have function below our lever of injury and above s5 and this known as an area of partial preservation. So we are ASIA A complete with an area of partial preservation.
So, here's my question, I am ASIA D from what it looks like. I have bladder retention though, doesn't that point to complete cauda equina syndrome? My doctors told me I was 'definitely incomplete' though..
